Transportation services are designed specifically to improve the participant’s and the family caregiver’s ability to independently access community activities within their own community in response to needs identified through the participant’s Person-Centered Plan. Subject to the hours approved and documented in the person-centered plan and agreed by the team. Transportation to/from outings such as medical appointments and personal appointments, as well as, any community-based activities such as shopping or church whenever necessary.
